After reviewing the issue it reminds me of the following document which could be a match for your issue:

Try the following steps in the instance where the patches were applied and provide feedback:
1. Login in Weblogic Console.
2. Select Services -> Datasources.
3. Click on EBSDatasource, and Press Lock & Edit.
4. Select Targets.
5. Select "forms_cluster1" and click Activate Changes.
6. Retest the issue and check whether users can now open forms successfully.
